1. Affirm will ask for information including name, email address, phone number, date of birth, and the last four digits of the social security number.
2. Affirm sends a text message to the customer’s cell phone with a unique authorization code to confirm the customer’s identity.
3. The authorization code will need to be added to the application form.

2. Enter the cell phone number. Affirm will send a personalized security pin code to the customer’s cell phone via text message.
3. Enter this security pin into the form and click sign in.
4. After signs in, a list of loans will appear with payments that are approaching. Click on the appropriate loan payment.
5. Make a payment using a debit card or ACH bank transfer. 

With our Healthy Guarantee, a customer is able to wear the shoes or Aetrex Orthotics for up to 30 days (from the date of delivery) and if not completely satisfied, the customer can return them.

Returns are free and simple to do, even when using Affirm as a payment! Simply use the postage-paid return label that was provided in your original shipment. Once your return shipment has been received and authorized, a refund will be processed within 10-14 business days in the original form of payment. Shipping charges from the original order cannot be refunded.
